Most problems can be sorted out quickly and easily with the person concerned, often at the time they arise, and this may be the approach you try first.

Where you are not able to resolve your complaint in this way, and wish to make a formal complaint, you should do so, in writing, as soon as possible after the  event and ideally within a few days.  This makes it easier to establish what happened.  A copy of our complaints policy is available here.

If you wish to make a complaint, please use the appropriate form which is available here.  When complete, please email to, or hand it in to our Seaforth Surgery at Vicarage Lane, Hailsham, for the attention of the Practice Manager.  If you are unable to download and print the complaint form, please contact the surgery on 01323 848494 and we will post a form out to you.
